Services

The Strive Rehabilitation @ Hawke's Bay has contracts with ACC, Ministry of Health and other agencies to provide Supported Living Services.

The service is for clients with acquired brain injury and with spinal cord injury who need support to get back into living at home and in the community.

Referral Process

Referrals to the Strive Rehabilitation @ Hawke's Bay Supported Living Service are usually made through ACC or Options Hawke’s Bay, rehabilitation practitioners, Brain Injury Association (HB) or the Stroke Foundation.

It is important that it is the client’s decision or preference to use the Strive Rehabilitation @ Hawke's Bay Supported Living Service and a commitment is made to the programme of work that is developed.

The following process is how a referral usually takes place though variations can occur such as self referrals or by family members.

  • An initial meeting with Strive Rehabilitation @ Hawke's Bay Supported Living Service Co-ordinator
  • The Strive Rehabilitation @ Hawke's Bay Supported Living Service Co-ordinator writes a service plan
  • Once the plan and hours of support are approved a Living Skills Coach will be introduced and work usually starts within 10 days of initial referral
  • Plans are reviewed at regular times

ACC –3 months and 6 months for active plans, 6 and 12 months for on going plans

Individualised Funding – 3 months, 6 months and 12 months

  • Closure reports are written at the end of each programme plan
  • Programmes can be changed or stopped at any time, just see the Strive Rehabilitation @ Hawke's Bay Supported Living Service Co-ordinator
